You control a hero who has a phantom twin, his alter ego. When the hero moves, the alter ego moves too in a mirrored fashion. This is a Genesis port of a ZX Spectrum homebrew game which was also released by Retrosouls. Retrosouls released the game digitally as Alter Ego: Remastered on both their website and their Itch.io page. Mega Cat Studios published physical carts in 2026 under the title AlterEgo: Dreamwalker, which is also the name of the ZX Spectrum sequel. Despite the name change, The Mega Cat release is the same game as the Remastered version that was released digitally. The title screen of every version of the game has the title as just "Alter Ego".
