Simple 2000 Series Vol. 58: The Gekai

Simple 2000 Series Vol. 58: The Gekai is a medical adventure game for PlayStation 2 that follows the life of a young surgeon working in the General Surgery Department of Seito University Hospital. The story begins when the protagonist is suddenly assigned to operate on a patient suffering from acute appendicitis, who turns out to be the case of a close colleague. As professional conflicts, ambition, and betrayal begin to surface within the surgical department, the protagonist must confront the question of what it truly means to be a doctor.

Gameplay combines visual novel storytelling with interactive surgical simulations. Players take direct control during thirteen different operations, including appendectomies and esophageal cancer resections, each based on real medical procedures. Every surgery must be completed under pressure, with limited time and the patient’s condition constantly at risk.

Successful operations improve the surgeon’s skill level and unlock special abilities. The story branches based on the player’s decisions and surgical performance, leading to eight unique scenarios and six possible endings. Through these choices, the game challenges players to balance ambition, morality, and compassion in the high-stakes world of modern surgery.
