Doki Doki Poyatchio!! tells the story of a young girl (default name is Jean) in a one month summer vacation (in a near futuristic world) that have to deliver food, letters and other items to the other characters in town. Her cousin Maria will be the one that will send him on those missions and the player have to deliver all the things she asked about. The characters of the town move around the map so is the player's duty to find them and solve the missions. The player's character's can also talk to the rest of the people of the town and move freely meanwhile he is delivering the items. When she solve some of the missions new places of the town will be opened to explore and find more people to deliver them other things.

The gameplay use a first person perspective and use close up views when the player characters appear and when they talk to other characters. The game features different events during the weeks.

The game features a first person perspective for the fishing part and 2 game modes: - Tournament Mode: Single play only, there the player can compete in a fishing tournament and try to win it. In this mode there are different fishing spots and tournaments to compete in, there is also a shop where the player can buy new rods, lures, etc using his fishing points. - Vs Fishing Mode: A 2 players split screen option for 2 human players to compete at the same time.
