Capitalizing on the vampire craze the world seems to be in, players of Dead End will control some high school kids and fight society’s new favorite villain. Each action in the game (such as “Go left” or “Open the door”) is represented by a numbered “paragraph” in the story, and players spend the game jumping from paragraph to paragraph. This means that there are multiple story branches, and players could play the game several times without even seeing all of the story. Jumping from paragraph to paragraph will also be used to solve puzzles within the game. Fights within the game involve dice rolls, with certain numbers corresponding to different attacks and skills for both the player and enemy.

While trying to profess his love to one of the heroines, Houdai finds himself being constantly chased by schoolgirls professing their love to him, either in written or screaming statements. There is no way to escape them, but you can fire pheromone projectiles at them which will give them ecstasy and create a passage for you to go through. During gameplay, the player can also use perspective zoom which aside from zooming can display through certain objects (e.g. you can see girl's bra through her t-shirt). For certain girls the action scene kicks in which lets you get more intimate as the camera is circling around the girl who is crazy for you, and you can only appease her appetite by touching or rubbing her... through the PlayStation Vita's touch screen feature, of course.

Keiichi learns that every year for the past four years, one person has been murdered and another has gone missing on the day of the Watanagashi Festival. Keiichi himself soon becomes drawn into the strange events surrounding the Watanagashi Festival and Oyashiro-sama. In each story arc, he or one of his friends become paranoid, and a crime is committed. Usually, the crime involves the murder of one of their own friends.
